Registration Forms
To secure a place at Fizzy Fish, we must have received a signed contract (this must state the sessions required) and a non-returnable £50 registration fee.
Before the child starts at the setting, we must have received and processed the Parental Consent Form plus the Child Record Form and Baby Information Sheet (if required). The last two forms need to be completed near the start date as they detail the child's routine, immunisations etc.
Please request the registration forms via the link below. Please return the forms, by hand, to 21 Tudor Close, Hove BN3 7NR or used the enclosed stamped addressed envelope. They should not be handed to either of the Pre-schools.
